The form below allows you to commit district or county office of education to the purchase of the CISI membership.

We encourage you to use our online system, but if you would rather mail or fax your membership information, you can simply fill out this form online and print the page, sending it to:

Billing policy: You will have immediate access to the CISI benefits. Billing for sign-ups on or before the 10th day of the month will be billed for that month. Billing for sign-ups after the 10th day of the month will begin the following month.
